User IDs and Password

All users of the Prevention Management, Reporting and Training System Web site are expected to adhere to the security standards of the Prevention Management, Reporting and Training System to the fullest, in respect to their interaction with other users and in handling substance abuse prevention information that Prevention Management, Reporting and Training System provides. The Prevention Management, Reporting and Training System issues each user a unique user ID and a system-generated password. As a user, you are required to change the system-generated password to a self-generated password. After 180 days the system will prompt you to change your password and if you fail to do so within the time allowed, you will be denied access to the Prevention Management, Reporting and Training System until you comply.

While attempting to access the Prevention Management, Reporting and Training System, you will be allowed only three login attempts, after which the Prevention Management, Reporting and Training System will freeze your user ID and password. If your user ID and password are frozen 12 times within a period of 12 months, you will be denied access to the Prevention Management, Reporting and Training System and your user privileges may be permanently revoked.


The undersigned gives the following assurances with respect to using Prevention Management, Reporting and Training System:


  • You will not allow any other person to share your user ID and password and will accept responsibility for all logins to the Prevention Management, Reporting and Training System Web site using your user ID and password.
  • You will not provide your user ID and/or password to any third party.
  • You will not leave the Prevention Management, Reporting and Training System Web site unattended while logged on to the system.
  • If you believe any breach of security has occurred, such as the disclosure, theft, or unauthorized use of your user ID and password, you will contact SAMHSA immediately.

Uploading and Downloading Data Sets


By uploading, inputting, providing and/or submitting any data sets, files and/or materials through Prevention Management, Reporting and Training System (each a "Submission," and collectively "Submissions"), you are granting all ownership rights in such Submissions to SAMHSA/CSAP, unless otherwise specified in writing. For all Submissions, you hereby grant SAMHSA/CSAP, its affiliates, and necessary sub licensees permission to use your Submission in connection with the operation of their businesses including, without limitation, the rights to: copy, download, reproduce, translate and reformat your Submission; and to publish your name, time and date in connection with your Submission. No compensation will be paid to you with respect to the use of your Submission, as provided herein. SAMHSA/CSAP will retain any data sets, files and materials uploaded, provided, and/or submitted through Prevention Management, Reporting and Training System for historical purposes and can use them at any time at its sole discretion. SAMHSA/CSAP, its affiliates, and necessary sub licensees reserve the right to follow-up with you to clarify and/or collect more supporting details regarding your Submission.


By posting, uploading, inputting, providing or submitting your Submission, you warrant and represent that you own or otherwise control all of the rights to your Submission as described in this section including, without limitation, all the rights necessary for you to provide, upload, input and/or submit the data sets, files, and/or materials.


The undersigned gives the following assurances with respect to the Prevention Management, Reporting and Training System data sets uploaded and/or submitted to SAMHSA/CSAP:


  • I will not leave my computer unattended at any time while I am logged on to Prevention Management, Reporting and Training System to upload and/or download data to SAMHSA/CSAP.
  • I will not leave any SAMHSA/CSAP completed questionnaire or material, electronic data files unattended at any time.
  • I will not use nor permit others to use Prevention Management, Reporting and Training System upload and submission to upload or submit unauthorized information to SAMHSA/CSAP.
  • I will not release any data sets, files or material entrusted to me to upload and submit through Prevention Management, Reporting and Training System to any public forum or unauthorized personnel.
  • I will not release nor permit unauthorized personnel to view any information that identifies persons, directly or indirectly for any data set, files or materials that I am uploading or submitting to SAMHSA/CSAP.
  • I will not attempt to use nor permit others to use the SAMHSA/CSAP data sets to learn the identity of any person included in any of the data sets.
  • I will not contact nor permit others to contact establishments or persons in the data sets to question, verify, or discuss data.
  • I will make no statement nor permit others to make statements indicating or suggesting that interpretations drawn are those of data suppliers or SAMHSA/CSAP.
  • I will acknowledge in all uploads and/or submissions based on the data sets, files and/materials that the source of the data is the “SAMHSA grantee not the individual in the data sets.”

Direct/Online Data Entry

  • I will acknowledge in all online data submissions that based on the data sets, files and/materials that the source of the data is the “SAMHSA/CSAP grantee not the individual in the data sets.”

Accessing Reports/Data Sets for Reports


Data Use Agreement for Data Coordination and Consolidation Center for CSAP/Prevention Management, Reporting and Training System (Prevention Management, Reporting and Training System)


***These data are currently only for use by CSAP staff and will not be available to the public until the public version of the site is activated. Public use is pending approval from SAMHSA counsel, FOIA, and confidentiality and privacy rights staff. These data have not yet been approved for public dissemination. ***


Under Section 501(n) of the Public Health Service Act (42 U.S.C. 290aa), data collected under Section 505 (42 U.S.C. 290aa-4) by the Substance Abuse and Mental Health Services Administration (SAMHSA) may be used only for the purpose for which they were collected. Under this authority, data supplied to SAMHSA under the auspices of the Data Coordination and Consolidation Center for the Center for Substance Abuse Prevention Services and Accountability Monitoring System (Prevention Management, Reporting and Training System) were supplied by the data sources only for the purpose of public health surveillance and associated statistical analysis of trends, groups, or categories of cases, not for investigations of specific individuals or organizations.


The statute states:


LIMITATION ON THE USE OF CERTAIN INFORMATION— No information, if an establishment or person supplying the information or described in it is identifiable, obtained in the course of activities undertaken or supported under section 505 may be used for any purpose other than the purpose for which it was supplied unless such establishment or person has consented … to its use for such other purpose. Such information may not be published or released in other form if the person who supplied the information or who is described in it is identifiable unless such person has consented … to its publication or release in other form.


Furthermore, Title V of the E-Government Act of 2002 (Pub. L. 107-347) applies similar restrictions to data collected for statistical purposes. In Section 512, it states:


(a) USE OF STATISTICAL DATA OR INFORMATION.—Data or information acquired by an agency under a pledge of confidentiality and for exclusively statistical purposes shall be used by officers, employees, or agents of the agency exclusively for statistical purposes.

(b) DISCLOSURE OF STATISTICAL DATA OR INFORMATION.

(1) Data or information acquired by an agency under a pledge of confidentiality for exclusively statistical purposes shall not be disclosed by an agency in identifiable form, for any use other than an exclusively statistical purpose, except with the informed consent of the respondent.

In addition, Section 513 of the E-Government Act of 2002 provides for stiff penalties for unlawful disclosures of information by employees of SAMHSA/CSAP and its sworn agents:


Whoever having sworn to observe the limitation imposed by Section 512, … and, knowing that … disclosure … is prohibited …, willfully discloses the information in any manner to a person or agency not entitled to receive it, shall be guilty of a class E felony and imprisoned for not more than 5 years, or fined not more than $250,000, or both.


Person identifiers – Any effort to determine the identity of any person or to use the information for any purpose other than for public health, analysis, and aggregate statistical reporting would violate the SAMHSA/CSAP statute (above) and the conditions of this data use agreement. Furthermore, under the statute, no identifiable information may be published or released if a person or establishment who supplied the information has not consented to its release. SAMHSA/CSAP does not collect direct personal identifiers. It may be possible in rare instances, through complex analysis and with outside information, to ascertain from the data sets the identity of particular persons. Considerable harm could ensue if this were done. By virtue of this agreement, the undersigned agrees that such attempts will not be made and that in any event such information would never be released or published.


The undersigned gives the following assurances with respect to the Prevention Management, Reporting and Training System data sets:


  • I will not use nor permit others to use Prevention Management, Reporting and Training System data in any way except for public health surveillance and associated aggregate statistical reporting.
  • I will require others in the organization (specified below) who use the data to sign this agreement and will keep those signed agreements on file and will submit copies of those signed agreements to SAMHSA/CSAP upon request.
  • I will not use, release nor permit others to release any information that identifies persons, directly or indirectly.
  • I will not use, release nor permit others to use or release the data sets or any part of them to any person who is not a member of the organization (specified below), except with the approval of SAMHSA/CSAP and the project officer for the grant programs under analysis.
  • I will not attempt to use nor permit others to use the Prevention Management, Reporting and Training System data sets to learn the identity of any person included in any set.
  • I will not contact nor permit others to contact establishments or persons in the data sets to question, verify, or discuss data in the Prevention Management, Reporting and Training System database.
  • I will make no statement nor permit others to make statements indicating or suggesting that interpretations drawn are those of data suppliers or SAMHSA/CSAP.
  • I will not use or release in any public forum any data or estimates from Prevention Management, Reporting and Training System that have not received prior approval for release by the U.S. Department of Health and Human Services and SAMHSA/CSAP.
  • I will acknowledge in all reports based on these data that the source of the data is the “Data Coordination and Consolidation Center, Center for Substance Abuse Prevention, Substance Abuse and Mental Health Services Administration.”

Public Data Sets


Data Use Agreement for Data Coordination and Consolidation Center of the CSAP/Prevention Management, Reporting and Training System (Prevention Management, Reporting and Training System)
