MFD Personnel Action Branch Article

Enlisted Reserve Soldier Services -PAB Functions-

Student Loan Repayment Program

  • Download a DD Form 2475 and complete Section 2.
  • Have the bank complete Section 3 of the DD Form 2475.
  • Submit the DD Form 2475 and a promissory note to the appropriate Personnel Actions Branch for processing. See PAB contact information at the end of this page.
  • The Personnel Actions Branch will complete Section 1 and submit the documents for action.
  • Once submitted the Personnel Actions Branch will contact the Soldier.

Retroactive Reserve Awards

  • Review the available Awards for Army Reserve Soldiers.
  • Determine the documentation needed to receive the award and submit a detailed request to the appropriate Personnel Actions Branch for action.
  • All supporting documentation should be in iPERMS unless it is a new document. IE: DA Form 638.
  • See "Uploading documents into iPERMS for IRR and AGR (above the line) Soldiers" for guidelines on how to submit documents to iPERMS.
  • Once complete the Personnel Actions Branch will contact the Soldier.

Issuing Certificate of Release or Discharge from Active Duty (DD Form 214)

  • If a DD Form 214 was not issued when the soldier separated from a period of Active Duty with the Army or Army Reserves.
  • Submit a detailed request to the appropriate Personnel Actions Branch for action.
  • Ensure that all supporting documentation is in iPERMS.
  • See "Uploading documents into iPERMS for IRR and AGR (above the line) Soldiers" for guidelines on how to submit documents to iPERMS.
  • Once complete the Personnel Actions Branch will contact the Soldier.
  • The DD Form 214 will be sent to iPERMS for upload.

Preparing Corrections to the DD Form 214 (DD Form 215)

  • Contact the Transition Center where the DD Form 214 was produced and request an update.
  • The Transition Center will email the request along with any supporting documentation to the appropriate Personnel Actions Branch for action.
  • Ensure that all supporting documentation is in iPERMS.

  • Once complete the Personnel Actions Branch will contact the Soldier.
  • The DD Form 215 will be sent to iPERMS for upload.

ERB updates for AGR Soldiers in above the line units

  • Submit a detailed request to the the appropriate Personnel Actions Branch.
  • Ensure that all supporting documentation is in iPERMS.

  • Once complete the Personnel Actions Branch will contact the Soldier.

Good Conduct Medals for AGR Soldiers in above the line units

  • Submit a detailed request for review of AGCM's to the appropriate Personnel Actions Branch.
  • Once complete the Personnel Actions Branch will contact the Soldier.
  • The document will be submitted to iPERMS.

Correction of Retirement Points

  • Submit a detailed request for review of retirement points to the appropriate Personnel Actions Branch.
  • Ensure that all supporting documentation is in iPERMS.

  • Once complete the Personnel Actions Branch will contact the Soldier.

Chronological Statement of Retirement Points (ARPC Form 249-E)

  • Submit a request for a ARPC Form 249-E to the appropriate Personnel Actions Branch.
  • Once complete the Personnel Actions Branch will contact the Soldier with directions on how to view the document online.
  • If the ARPC Form 249-E is found to be incomplete the Soldier should submit a request for "Correction of Retirement Points."

Computation of PEBD, BASD, and Active Federal Service (DA Form 1506)

  • Submit a detailed request for a DA Form 1506 to the appropriate Personnel Actions Branch.
  • Ensure that all supporting documentation is in iPERMS.
  • See "Uploading documents into iPERMS for IRR and AGR (above the line) Soldiers" for guidelines on how to submit documents to iPERMS.
  • Once complete the Personnel Actions Branch will send a DA Form 1506 to the Soldier for review.
  • Print the DA Form 1506 and sign block 11A indicating you have reviewed the document.
  • Sign block 18 item 1 or 2 indicating if you concur with the document or not.
  • If you do not concur with the DA Form 1506 provide documentation for updates and submit it with the DA Form 1506.
  • Submit the completed document to the appropriate Personnel Actions Branch.
  • Once complete the Personnel Actions Branch will send the Soldier a copy of the authenticated DA Form 1506.
  • The final DA Form 1506 will be sent to iPERMS for upload.

Employment Verification

  • The requesting entity should complete a Request Pertaining to Military Records or Standard Form 180 (SF-180).
  • Submit the request to the appropriate Personnel Actions Branch along with any signed releases obtained by the employer.
  • Once complete the requested documentation will be mailed to the requestor.

Uploading documents into iPERMS for IRR and AGR (above the line) Soldiers

  • The Personnel Actions Branches only submit documents to iPERMS for IRR,AGR (above the line), and Gray Area Retirees.
  • All other Soldiers have a Unit Administrator or S1 to submit their documents.
  • Submit a detailed request to the appropriate Personnel Actions Branch along with copies of all documents.
  • Clearly indicate if the documents are for a Board so that they are given priority for upload.
  • Allow up to 90 days for the documents to be available for viewing. Board documents are viewable 7-10 days after submission.
  • Review the list of documents authorized for upload.

Army Review Board Agency (ARBA) Inquiries

  • If you feel that an error or injustice exists in your military records and you have exhausted all other administrative remedies available, you may apply for corrections to the Army Board for Correction of Military Records or to the Army Discharge Review Board.
  • You can obtain application forms and information on the Army Review Boards Agency (ARBA) web site.

VA Home Loan Statement of Service

  • Reservists or veterans must have at least six qualifying years of service in the Reserve.
  • Active Duty soldiers must have completed 90 consecutive days or more or 24 months of Active Duty (AD) or Inactive Active Duty Training (IADT). The exact length requirement depends upon when you were on Active duty.
  • Submit a Standard Form 180 to the appropriate Personnel Actions Branch.
  • Request a ARPC Form 606-E, DD Form 214 if available, and discharge orders if available.
  • Soldiers with previous service in the ARNG should request a NGB 23 from their previous ARNG unit.

Records Requests

  • The requesting organization should complete a Request Pertaining to Military Records or Standard Form 180 (SF-180).
  • Submit the request to the appropriate Personnel Actions Branch.
  • Once complete the requested documentation will be mailed to the requestor.

Update Army Physical Fitness Test (DA Form 705)

  • Submit a completed DA Form 705 to the appropriate Personnel Actions Branch.
  • Once complete the Personnel Actions Branch will contact the Soldier.

Update the Reserve Data Maintenance System/TAPDB-R

  • Submit a detailed request to the appropriate Personnel Actions Branch.
  • Ensure that all supporting documentation is in iPERMS.
  • See "Uploading documents into iPERMS for IRR and AGR (above the line) soldiers" for guidelines on how to submit documents to iPERMS.
  • Once complete the Personnel Actions Branch will contact the Soldier.

Name Change, Social Security Number Correction or Date of Birth Correction

  • Submit a detailed request to the appropriate Personnel Actions Branch along with the following documentation.
  • Name change documents (e.g., marriage certificate, divorce decree if decree states change of name or court order)
  • SSN documents (e.g., copy of Social Security card)
  • Date of Birth documents (e.g., birth certificate or authenticated copy)

Transfers from the Retired Reserve to the IRR/TPU/IMA

  • Submit a detailed request to the appropriate Personnel Actions Branch along with the following documentation.
  • DA Form 4187 or DA Form 4651 completed by the gaining unit stating the UIC, paragraph, and line number of the position.
  • All Physical and MEDPROS documentation from the soldiers separation, if available.
  • You must be eligible for award of the appropriate level of security clearance.
  • Your TPU assignment requires approval from the unit commander or his or her designated representative. 

Transfers to the Retired Reserve from the IRR/IMA

  • IRR Soldiers may submit a request for transfer to the Retired Reserve to the appropriate Personnel Actions Branch.
  • IRR Soldiers do not need to provide further documentation.
  • IMA Soldiers need to submit their request through their Chain of Command. Chain of Command will then process the request through the PAB.
