Consolidated Health Centers
Consolidated Health Centers (CHCs) provide health services to underserved populations which include all residents in their catchment area, regardless of ability to pay. This includes people who face barriers in accessing services because they have difficulty paying for services, have language or cultural differences, or because there is an insufficient number of health professionals/resources available in their community.
Health Centers provide health care services as described in statute and regulation. They provide basic preventive and primary health care services. Health Centers also provide services that help ensure access to primary care such as case management, outreach, transportation and interpretive services. Fees are discounted or adjusted based upon the patient’s income and family size from current Federal Poverty Guidelines. All grantees must demonstrate that all persons will have access to the full range of required primary, preventive, enabling, and supplemental health services, including oral health care, mental health care and substance abuse services, either directly on-site or through established arrangements.
Eligible Applicants:
Public and private nonprofit healthcare organizations are eligible to receive funding for this program. Funds may be awarded for new service delivery site, expansion of medical capacity, and expansion of services to include oral health and pharmacies.
Eligible Beneficiaries:
Primary beneficiaries of this program are low-income individuals and families who are currently underserved – special populations include migrant or seasonal farm workers, those living in public housing, and those experiencing homelessness.
Funding Mechanism:
Grants are awarded by the Health Resources and Services Administration competitively to eligible applicants for initial funding. Only existing grantees are eligible to apply for expansion funding of any type.
