When you marry or remarry, you may enroll, change from Self Only to Self and Family, or change from one plan or option to another. You must submit your enrollment change from 31 days before to 60 days after the change in family status.
Family members eligible for coverage under your Self and Family enrollment include your spouse (including a valid common law marriage) and unmarried dependent children under age 26 (including stepchildren) and foster children who live with you in a parent-child relationship.
If you have a Self and Family enrollment, you do not have to complete a new Health Benefits Election Form (SF 2809). You should contact your health plan to inform them of the new family member(s). If your health plan questions whether someone is an eligible family member, the health plan may ask you or your Human Resources or Retirement Office for more information.
