National Forum for Heart Disease and Stroke Prevention

In 2003, the American Heart Association, the Association of State and Territorial Health Officials, and the Centers for Disease Control and Prevention founded the National Forum for Heart Disease and Stroke Prevention to provide leadership for implementation of A Public Health Action Plan to Prevent Heart Disease and Stroke, a national plan to chart a course for the prevention of heart disease and stroke.

Today, the National Forum acts as a catalyst to drive multi-sector action to prevent heart disease and stroke. National Forum members include stakeholders from the non-profit, for-profit, and government sectors. The organization leads collaborative policy and programmatic efforts as well as builds the collective voice for the prevention of heart disease and stroke.

The National Forum will support Million Hearts™ by convening, catalyzing and coordinating Member activities supporting ABCS best practices, the establishment of a comprehensive cardiovascular surveillance system, and improving the health equity among all Americans.
