Self-Measured Blood Pressure Monitoring Action Guide

Clinicians, public health practitioners, health care systems, and individuals can focus on disease management strategies to improve blood pressure control in order to improve health outcomes for patients with HTN. Self-measured blood pressuring monitoring (SMBP) is one such strategy that is being promoted by Million Hearts™ and numerous national and international health organizations. The SMBP guide provides guidance and resources for public health practitioners on the integration of self-measured blood pressure monitoring with clinical feedback into chronic disease prevention efforts.

Interactive Atlas of Heart Disease and Stroke

Interactive Atlas of Heart Disease and StrokeThe Interactive Atlas of Heart Disease and Stroke is an online tool that enables public health professionals and concerned citizens to document geographic disparities in heart disease and stroke at the local level. Users can create county-level maps of heart disease and stroke by gender, race/ethnicity and age group, and overlay maps with congressional boundaries and locations of health care facilities. In addition, maps showing county-level poverty, education, the food environment, access to health care, and other social factors are available. The maps in the Atlas can be used to tailor heart disease and stroke prevention programs and policies to the needs of specific communities, and target scarce resources to communities with the heaviest burdens.

You can access the Interactive Atlas of Heart Disease and Stroke at http://apps.nccd.cdc.gov/DHDSPAtlas.
