Community Profiles

Fifty community–level grantees are implementing tailored efforts—based on each community's unique health needs and composition—to tackle obesity and tobacco use. Individual communities are closely attuned to the health-related challenges of their residents, and CPPW provides them with the resources, flexibility, and technical support to address those challenges.

Each of the 50 communities—a mix of urban, suburban, rural, and tribal areas—is focusing on obesity prevention, tobacco use prevention, or both. See below for community profiles that detail how each community is implementing environmental change to make healthy living easier.
