Utah CHIP Program
Program Description
The Utah Children's Health Insurance Program, or CHIP, provides medical services to eligible children under the age of 19 who do not have health insurance and do not qualify for Medicaid. Services provided under the CHIP program include: sick visits; well child exams; prescriptions; dental; hospital care; emergency room visits; urgent care visits; eye exams, and related services. CHIP emphasizes preventive care and there are no co-payment requirements for any preventive services (e.g., well-child checkups and immunizations). There are some co-payments required for other services and quarterly premiums. All costs are dependent on the family's income. CHIP is administered by the Utah Department of Health, and currently contracts with the Public Employees Health Program (PEHP) and Molina to provide health care services. Because of limitations on funding and available slots in the program, CHIP only accepts applications during open enrollment periods. In order to qualify for this benefit program, you must be a resident of Alaska, under 19 years of age or be a primary caregiver with a child under the age of 19 or be pregnant, not covered by health insurance (including Medicaid), a U.S. national, citizen, legal alien, or permanent resident.Depending on your income level, you may have to pay a premium for coverage.
