Information Collected and Stored Automatically

If you do nothing during your visit but browse through the Web site, read pages, or download information, we will gather and store certain information about your visit automatically. If You Send Us E-mail

If You Fill Out Online Forms

The CME/CEU course registration and course evaluations include forms where you actively enter your personal information. Only health care providers who require continuing education credit may need to provide personal information. We use this personal information to provide health care providers with accurate continuing education credit. Specifically:

• When health care providers register, we need to know their name, specialty, E-mail address, and mailing address. This allows us to process and fulfill ACCME requirements.
• When health care providers complete the course evaluation, we need to collect their opinions on the course so that we can process the continuing education credit and fulfill ACCME requirements. Course participants do not have to provide non- required information but it will help us to more accurately meet your educational needs.
• We may also use the information we collect to inform you of new NCCAM educational programs.

Cookies

Some pages on this Web site employ "cookies." A cookie is a small file that a Web site stores on your computer to maintain a session. The cookies and the information about your session are automatically destroyed shortly after you close your browser. The information is not permanently stored on your computer.
