The Data Bank E-Authentication Process


New User Registration

  1. The Entity Data Bank Administrator enters ID and email address for the prospective user.
  2. After receiving electronic notification, the prospective user (employee) completes user account information online.

Is the employee at the Administrator's location? If they are at the different locations, see Table 1. If they are at the same location, skip to Table 2.

 3. User prints and completes the User Registration document.
 4. Notary Public certifies the User Registration document.
 5. User mails document to Data Bank Administrator.
6. The Administrator verifies employee (user) employment. 
7. If the Data Bank Administrator does not approve the User Registration document, the user will receive electronic notification and can begin again at Step 2. If the Administrator approves it, he should mail the User Registration document to the Data Bank.

Note: Initial user approval by the Data Bank Administrator is temporary approval, contingent on the Data Bank receiving documents within 15 days. The Administrator should provide the new user with the organization's Data Bank Identification Number (DBID) so that he can begin using the Data Bank system.
 8. If the Data Bank does not approve the User Registration document, the registrant will receive electronic notification and can begin again at Step 2. If the Data Bank authorizes the user, he has final approval to continue using the Data Bank system.

 3. User prints and completes the User Registration document.
4. The Administrator witnesses the registrant's signature on the User Registration document. 
5. Administrator verifies the registrant's employment. 
6. If the Administrator does not approve the User Registration document, the registrant will receive an electronic notification and can begin again at Step 2. If the Administrator approves the user, he should mail the User Registration document to the Data Bank.
