Form Search Tips
Enter search information in only one box.
By number: If you know the Form Number, enter it in the top box using the digits (and letters if appropriate) after the prefix. You do not have to use a prefix to search. The search engine will find all forms containing the number you enter. It will accept as few as 1 digit for search purposes.
By title: If you know the Form Title, enter one or two words from the Form Title in the lower box.
By keyword: If you do not know either the number or title, you can also search for a form by entering one or two words that describe the benefit or service for which you need the form. Enter the key word in the lower box.
