- Undergraduate Institution Certification for Exceptional Financial Need (EFN) Form (102KB PDF File) - FAFSA information used to complete this form must be taken from 2012 taxes. Section A of this form may be completed prior to printing. Please use this option so your name, student number, and e-mail address are readable.

- Applicaiton Directions - The UGSP will have a new application system for Fall 2013 admission consideration. Application directions for the new system will be provided at a later date.
