CME Information

Course Overview

An increasingly recognized option for managing selected cases of acute respiratory failure is noninvasive positive pressure ventilation (NPPV). The use of NPPV for support during the treatment of respiratory failure is attractive because it does not require either endotracheal intubation or sedation and can be easily initiated or discontinued as needed. It is also associated with few of the nosocomial complications recognized with endotracheal intubation. However, NPPV is a resource-intensive modality, requiring substantial training and experience for its successful implementation.

NPPV has been evaluated in a large number of trials, often with clinically important benefits. However, the use of NPPV remains highly variable across and within countries. Challenges related to the use of NPPV include a lack of physician knowledge, low rates of perceived efficacy.

This CME activity covers the systematic review of the current clinical literature, conducted by the Duke Evidence-based Practice Center, to evaluate the benefits and/or harms of using NPPV in the management of patients with acute respiratory failure of various etiologies.

Educational Objectives

At the conclusion of this activity, the participant should be able to:

  • Recognize the role of noninvasive positive pressure ventilation (NPPV) in managing patients with acute respiratory failure and the key modes of applying NPPV in this patient population.
  • Describe the current evidence regarding the benefits and harms associated with application of NPPV versus supportive care or invasive ventilation in managing patients with acute respiratory failure.
  • Interpret the current evidence regarding the benefits and harms of NPPV with bilevel positive airway pressure (BPAP) versus continuous positive airway pressure (CPAP) in managing patients with acute respiratory failure.

Target Audience

This CME activity is designed to meet the educational needs of emergency medicine specialists, critical care specialists, and, intensivists.
