Once you have been asked by an FBI representative to complete the FBI Background Investigation Forms, you will complete several forms: the SF-86 and the FD-979. Please review the following instructions carefully to ensure that you fully complete all of the required forms.

About the SF-86

The SF-86 (Questionnaire for National Security Positions) is a standard background investigation form used by the entire U.S. Intelligence Community. After you have been selected for a position, you will be invited to fill out the SF-86 form using the Office of Personnel Management's Electronic Questionnaire for Investigations Processing (e-QIP) system. The forms listed below are provided in .pdf form for review only. Please ensure that you complete the entire SF-86 for the most recent 10 year period or back to your 18th birthday, whichever is shorter. You must provide at least two years of information, regardless of when your 18th birthday occurred.

Please complete the appropriate portions of the SF-86:

Guidelines for Completing the SF-86

• Be completely honest and forthright when answering all questions. If necessary, provide any clarification or explanation for how you answered a particular question in the comments or continuation section.

• If you resided in an apartment complex, include the name of the complex and the unit number. If your name was not on the lease, include the name of the individual who was on the rental/lease agreement.

• If you have lived overseas for over six months or more, provide references currently in the United States who can corroborate or verify your overseas activities. If necessary, add this information in the continuation section of the SF-86.

• In Section 13, ensure that you provide a response to the "Reason for Leaving" for all prior employments. If you had a period of unemployment or were a full-time student, list those periods as "Unemployment" (Code 7), along with the contact information for someone (non-relative) who can verify your activities during that time.

• If you do not know your Selective Service System (SSS) registration number for Section 14, you can obtain it by calling the automated system at 1-847-688-6888 or via the Internet at www.sss.gov.

• For question 18 on the SF-86, for any relatives or associates residing in foreign countries (if in doubt, list them), provide the following information either in the continuation section of the SF-86 or on an attached piece of plain bond paper that is submitted with your signed release forms: (a) the person's full name and occupation; (b) current employer; (c) whether they now or in the past have worked for a government agency, police, security or intelligence organization, and if so, which specific agency/organization and government(s); and (d) the type and frequency of contact you have with them (e.g., phone/monthly, email/weekly, in person/annually, etc.) as well as the date of the your last contact.

• For section 25, include information about any prior security clearances issued. If uncertain of the type(s) or date(s), at a minimum provide the agency that granted the clearance(s).

• As indicated in the "Disclosure Information" section of the SF-86, the information on this form and information collected during an investigation, may be disclosed without your consent as permitted by the Privacy Act and published routine uses. As provided for by the SF-86 Privacy Act Routine Use Number 3, when a record on its face, or in conjunction with other records, indicates a violation or potential violation of law, the relevant records may be disclosed to the appropriate Federal, foreign, State, local, tribal, or other public authority responsible for enforcing, investigating or prosecuting such violation. Notwithstanding any language to the contrary, your response to question 29f may be disclosed pursuant to the above. About the FD-979, the DOJ-555 and the FD-1052

The FD-979 authorizes the FBI to obtain information on you from other organizations (e.g., academic institutions, previous employers, credit bureaus, etc.). The DOJ-555 allows the FBI to obtain one or more consumer/credit reports. The FD-1052 is an agreement of your pledged loyalty to the U.S. if you possess dual citizenship.
