Can I get a copy of my Form W-2 from Social Security?
We charge $38 per copy for each year you need a copy. If you need the copy to be certified, it costs an additional $32, for a total of $70 each.
To request a copy of your Form W-2, send us:
- Your Social Security number;
- Your name exactly as shown on your Social Security card;
- Any different names shown on your Form W-2;
- Your complete mailing address;
- Year(s) for which you need a copy; and
- Your telephone number.
Mail your request along with the correct fee to:

You can also request a copy of your Form W-2s from the Internal Revenue Service (IRS) using the Request for Copy of Tax Return (IRS Form 4506).
