GRANTS

The Office of Minority Health (OMH) administers grant programs in an effort to facilitate community linkages and strategies that use scarce resources efficiently across national lines. These grants are awarded to support community organizations and science-based efforts to eliminate health disparities.

2012

• Peer-to-Peer Outreach Model to Curb HIV/AIDS Transmission Among High Risk Minority Youth and Adolescents (CHAT)
  The CHAT program seeks to improve the HIV/AIDS health outcomes of high risk minority youth (13-19 years old) by supporting community-based efforts to increase HIV/AIDS prevention/education efforts, testing, counseling and referrals. Deadline: June 8, 2012
  
  
• Eliminating Lupus Health Disparities Initiative (ELHDI)
  The purpose of the ELHDI is to eliminate lupus related health disparities among racial and ethnic populations through the dissemination of a national lupus education program to health professionals and student trainees in targeted schools and health care systems/practices. Deadline: June 8, 2012
  
  
• HIV/AIDS Health Improvement for the Re-entry Population Demonstration Program (HIRE)
  Through the HIRE Program, OMH seeks to improve the HIV/AIDS health outcomes of formerly incarcerated persons re-entering the mainstream population by supporting community-based efforts to ensure their successful transition from state or federal incarceration back to their communities. Deadline: June 8, 2012
  
  
• Youth Empowerment Program (YEP)
  The YEP seeks to address unhealthy behaviors in at-risk minority youth, 10-18 years of age, and provide them with opportunities to learn skills and gain experiences that contribute to more positive lifestyles and enhance their capacity to make healthier life choices. Deadline: June 8, 2012
  
  
• American Indian/Alaska Native Health Disparities Program (AI/AN)
  The overall intent of the AI/AN program is to strengthen the capacity of Tribal Epidemiology Centers (TECs) and Urban Indian Health Programs (UIHPs) to: collect and manage data more effectively; better understand and develop the link between public health problems and behavior, socioeconomic conditions, and geography; and create a pipeline program for students to increase racial and ethnic diversity in the public health and biomedical sciences professions. 2010

• $2.3 Million Available for Health Disparities Umbrella Cooperative Agreements
  $2,275,000 is available from OMH for partnerships to address: access to care; achieving health equity by addressing social determinants of health; youth violence (including gang violence); work force development; and data collection, analysis, and evaluation. Deadline: August 2, 2010.
  
  
• $4 Million Available for Health Equity Community Based Networks
  $4 million is available from OMH for community-based networks that employ evidence-based strategies; address social determinants and environmental barriers to access; and increase access to and utilization of preventive health care, medical treatment, and supportive services. Deadline: August 2, 2010.
  
  
• $1 Million Available for Prevention among College Students
  $1 million is available from OMH for partnerships seeking to improve the health status, relative to HIV/AIDS, of young adults in racial/ethnic minority groups. Deadline: August 2, 2010.
  
  
• $2.8 Million Available for Resource Networks for Families Affected by HIV/AIDS
  $2.84 million is available from OMH for Health and Social Service Resource Networks equipped to work with high-risk minority families who are affected by HIV/AIDS and are in transition from incarceration, domestic violence, and/or substance abuse treatment. Deadline: August 2, 2010.
  
  
• $6 Million Available for Grants to State and Territorial Offices of Minority Health
  $6.45 million is available from OMH for grants to state and territorial offices of minority health to demonstrate the effectiveness of strategic partnerships in improving minority health and eliminating health disparities.

2009

• $250,000 Available for Hispanic Physicians’ Leadership Development Initiative
  Approximately $250,000 is available to support one competitive cooperative agreement that will establish a consortium of institutions to promote healthy lifestyles among Hispanics and other underserved minority populations. Deadline: September 10, 2009.
  
  
• $3.5 Million Available for Youth Empowerment Programs
  Some $3.5 million is available to support competitive cooperative agreements for youth empowerment programs. Institutions of higher education, community colleges and Tribal colleges may apply for projects that support positive lifestyles and healthier life choices among youth aged 10-18. Deadline: September 3, 2009.
  
  
• New OMH Funds Target HIV/AIDS Outcomes for Ex-Offenders
  OMH is re-announcing the availability of $2 million for 8 to 10 new, competitive, cooperative agreements under the HIV/AIDS Health Improvement for Re-entering Ex-Offenders Initiative. Organizations that submitted applications accepted by June 12 need not re-apply.

• $2.5 Million Available for HIV Projects for Youth
  Some $2.5 million is available for competitive HIV/AIDS cooperative agreements that will support prevention and testing for at-risk adolescents, employing technology-based approaches, social networking web sites, and peer educators.

• Funds Available for Eliminating Disparities in Lupus
  Some $1.6 million is available for a cooperative agreement to improve early diagnosis, treatment, and management of lupus for populations disproportionately affected. The project is expected to result in a consortium to expand and promote the utilization of more comprehensive lupus curricula in medical and nursing schools, and among health care professionals and professional associations.

• Region VII Announces Healthy Baby Funds Available
  The HHS Region VII OMH announced the availability of funds for 6-8 competitive contracts to support efforts to decrease infant mortality in Iowa, Kansas, Nebraska or Missouri. Contracts will not exceed $500.

• Funds Available for Minority Health Institute in the Delta
  Approximately $5.3 million is available for a cooperative agreement to improve the health and well-being of racial and ethnic minorities in the Mississippi Delta Region.

2008

• OMH Invites Applications for HIV/AIDS Capacity Development Grants
  There is $3 million available for OMH competitive grants as part of the Collaborative Technical Assistance and Capacity Development Demonstration Grant Program for the Coordination and Delivery of a Continuum of HIV/AIDS-Related Services in Minority Communities (CTA/CD). Twelve to 15 grant awards are expected.

2007

• OMH Invites Applications for AI/AN Health Disparities Program Grants
  Approximately $1 million is available for new, competitive grants under OMH's American Indian/Alaska Native Health Disparities Program.

2006

• Youth Empowerment Demonstration Grant Program
  This program is designed to address unhealthy behaviors in at-risk minority youth, and provide them opportunities to learn about more positive lifestyles and enhance their capacity to make informed and healthier life choices. It is intended to test community-based interventions on reducing risky behaviors among targeted minority youth. These demonstration grants require a multi-partner approach involving institutions of higher education, primary and secondary schools, community organizations and institutions, and the community at-large.

• Minority Community Health Partnership HIV/AIDS Demonstration Grant Program
  This program seeks to improve the health status relative to HIV/AIDS, of targeted minority populations through health promotion and education activities. This program is intended to test community-based interventions on reducing HIV/AIDS disparities among racial and ethnic minority populations, and demonstrate the effectiveness of community-based partnerships involving non-traditional partners at the local level.

• Mississippi Institute for Improvement of Geographic Minority Health and Health Disparities Program
  This program is designed to address the many and significant health disparities faced by rural disadvantaged and minority populations throughout the state. This program is intended to demonstrate the effectiveness and efficiency of a targeted and multifaceted statewide approach for eliminating these health disparities.

• State Partnership Grant Program to Improve Minority Health
  As part of a larger national strategy, this new initiative seeks to facilitate the improvement of minority health and elimination of health disparities (adult/child immunization, asthma, cancer, diabetes, heart disease and stroke, HIV, infant mortality, and mental health) through the development of partnerships with established state and territorial offices of minority health. • Technical Assistance and Capacity Development Demonstration Grant Program for HIV/AIDS-Related Services in Minority Communities
  [CFDA Program No. 93.006]
  Grant recipients will identify community-based minority-serving organizations that are well linked with minority populations affected by HIV/AIDS, and which have recognized needs and/or gaps in their capacity to provide HIV/AIDS-related prevention and care services. This will be facilitated by providing administrative and programmatic technical assistance; and assisting those community-based minority-serving organizations, through an ongoing mentoring relationship, in the development of their capacity as fiscally viable and programmatically effective organizations to successfully compete for federal funds and other resources.

• Programs To Improve Minority Health and Racial and Ethnic Disparities in Health
  This funding includes three programs for FY 2004: (1) Community Programs to Improve Minority Health; (2) Bilingual/Bicultural Service Demonstration Grant Program; and (3) HIV/AIDS Health Promotion and Education Program. COOPERATIVE AGREEMENTS

• HIV/AIDS Health Promotion and Education Cooperative Agreement Program
  [CFDA Program No. 93.137 - Formerly: Minority Community Health Coalition Demonstration Program, HIV/AIDS
  These grants are aimed at increasing community understanding of HIV/AIDS and improving access to HIV/AIDS counseling, testing, and treatment services. The involve coalition efforts to promote integrated community responses; address sociocultural, linguistic, and other barriers to effective HIV prevention and services, to increase the number of individuals seeking and accepting treatment; and develop and conduct education and outreach efforts for hardly reached populations.

• Family and Community Violence Prevention Program
  The Family and Community Violence Program (FCVP) was developed in fiscal year 1994 under a cooperative agreement between OMH and Central State University (CSU), Wilberforce, Ohio. The purpose of the FCVP is to impact the increasing incidence of violence and abusive behavior in low income, at-risk communities through the mobilization of community partners. The FCVP coordinates the activities of Family Life Centers (FLCs) at Historically Black Colleges and Universities, Hispanic Serving Institutions, Tribal Colleges and Universities, and other minority-serving institutions in 17 states, the District of Columbia, Puerto Rico and the U.S. Virgin Islands. The FLCs are engaged in violence prevention activities which address the academic, personal, cultural and career development of youth who are at risk for involvement in violent and other abusive behavior. Each FLC provides a summer academic enrichment program and conducts activities to improve family bonding, parenting skills and social interaction skills. Each has collaborative agreements with schools and community organizations to assist in recruiting youth and their families and delivering services.

• Community Initiatives to Eliminate Stroke Program (CITIES)
  The Community Initiatives to Eliminate Stroke Program (CITIES) is intended to increase community awareness and knowledge of hypertension and stroke, enhance early detection of high blood pressure and stroke with early referral to care, increase the community's adoption and use of lifestyle behaviors known to promote prevention and control of hypertension and stroke, reduce the death rate by stroke in targeted communities, and enhance blood pressure control rates among community persons who are known to have hypertension and who are members of a health plan or otherwise visit health systems, clinics, or medical offices. The three grantees are: Medical University of South Carolina, Charlestown, SC; Forsyth Medical Center Foundation, Winston-Salem, NC; and University of Alabama at Birmingham, Birmingham, AL. (2005)

• National Umbrella Cooperative Agreement Program
  The National Umbrella Cooperative Agreement Program seeks to increase the diversity of the health-related work force, reduce health disparities and improve quality of care for targeted minority populations through projects that are of national significance, and improve evaluation procedures and the collection and analysis of data on targeted minority populations. This program offers a mechanism for the Department and other Federal agencies to colaborate on projects addressing these purposes. Nine organizations were awarded cooperative agreements in FY 05, four of which focus on health disparities projects and five on work force development projects.
