Collaborative Approach to CKD Care

Appropriate care for patients diagnosed with chronic kidney disease (CKD) requires collaborative, interdisciplinary care including all the health professionals in the primary care setting. Collaborative management by primary care providers and consultants like nephrologists and registered dietitians (RDs), can help maintain continuity of care and improve outcomes. However, it is imperative that all members of the health care team communicate effectively to facilitate collaborative care.

Collaborate with a Nephrologist

Although much of the care necessary for patients with CKD can be managed by a primary care provider, referral to a nephrologist for consultation or to transfer care is usually necessary with disease progression. Learn more about when it may be appropriate to consult with a nephrologist and how to facilitate better collaborative care using the Nephrology Referral Form.

Collaborate with a Registered Dietitian

Diet therapy is critical for patients with CKD. While the primary care provider can and should initiate a discussion about dietary changes with CKD patients, it is vital to engage and refer to a RD knowledgeable in CKD diet and nutrition to assess and conduct individualized medical nutrition therapy (MNT) for patients with CKD. Learn more about dietitian referral and how to maximize the benefits to your patient by using the CKD Diet Counseling (MNT) Referral Form.
