Advanced Trauma Life Support® (ATLS®) is a 2 1/2 day course sponsored by the American College of Surgeons©, (ATLS®) prepares physicians to identify and respond to life-threatening traumatic injuries. Additionally, (ATLS®) is not a stand-alone exportable course; it is conducted in conjuntion with C4.
The content and skills presented in this course are designed to assist doctors in providing emergency care for the trauma patient. The ATLS Course provides the essential information and skills doctors may apply to identify and treat life-threatening or potentially life-threatening injuries under the extreme pressures associated with caring of patients in the fast-paced environment and anxiety of the resuscitation room.

COMBAT CASUALTY CARE COURSE (C4)
C4 is a basic 8-day tri-service continuing medical education program designed to enhance the operational medical readiness skills of physicians, nurses, physician assistants, dentists, and other specialty healthcare providers intended for initial entry medical professionals with minimal field exposure. C4 prepares junior tri-service medical officers with the knowledge critical in conducting Level I & II healthcare operations in an austere, combat environment. C4 provides training in leadership skills, field medical knowledge and the practical information needed for direct medical support of tactical units under combat conditions. The course includes 3 days of professional programs at the Defense Medical Readiness Training Institute, Fort Sam Houston and five-days of intensive field training at Camp Bullis, Texas.
Students attending C4 have the opportunity to receive certification in Advanced Trauma Life Support® (physicians and oral surgeons; dentists, physician assistants,and nurse practitioners may attend as auditors), the Trauma Nursing Core Course (nurses) and Pre-Hospital Trauma Life Support (other healthcare providers). Participants are awarded continuing education/credentials upon successful completion of these courses.
At the conclusion of the professional programs, the course moves to the field as students rotate through various training stations in an operational environment. Areas of instruction are varied and complex, ranging from bandaging and splinting to Level 1 & Level 2 care in the Tactical Simulator for Military Medicine (Tac-SiMM). Students encounter scenarios simulating combat environments where they must perform the duties of their respective career-field positions as well as the duties of first responders. They also have exposure to various real-world scenarios, as well as a myriad of medical readiness topics taught through didactic lectures.

EMERGENCY WAR SURGERY COURSE |
The Emergency War Surgery Course (EWSC), formerly known as the Trauma Refresher Course For Surgeons (TRCS), establishes combat trauma training competencies and coordinates training to develop and sustain DoD trauma surgeons whether located in an operational environment, military MTF or at a Level I trauma treatment facility.
Intended for general and orthopedic surgeons, the course was developed in response to 1991 Gulf War reports that surgeons and other healthcare providers lacked sufficient training and experience in trauma surgery. To remedy this, the Deputy Assistant Secretary of Defense for Health Affairs designated proficiency in combat surgery training as an “essential mission.” The Combat Trauma Surgical Committee (CTSC) was created in 1996 to meet this mission and to coordinate training efforts to develop and sustain DoD trauma care providers.
The EWSC is a standardized, measurable and reproducible training program that will give surgeons and other health providers exposure to critical life saving skills necessary to sustain the force, fight and win in the 21st century.

JOINT OPERATIONS MEDICAL MANAGERS COURSE |
JOMMC familiarizes and develops senior healthcare personnel to serve as Joint Task Force Surgeons’ and Staff members. The mission of these individuals is to:
- Provide Health Service Support (HSS) recommendations to the JTF Commander
- Coordinate HSS operations in the Joint Operations Area
- Establish and maintain operational medical facilities
- Coordinate Coalition and Civilian medical interoperability
JOMMC is made up of just-in-time training to provide Joint HSS instruction punctuated with scenario-based tabletop exercises that reinforce course objectives.
JOMMC is intended to provide entry level instruction on Joint HSS. JOMMC is not intended for those students who have previously attended the JTF Surgeon Course or Joint Medical Planners Course (JMPC).
JOMMC is open to Tri-Service Active Duty, Reserve, Guard, US Public Health Service, and US Coast Guard medical department personnel, as well as foreign national medical department officers. Students will leave the course with the capability to provide doctrinally sound, operationally integrated leadership in joint health service support operations successfully meeting current demands on combat service support units. MEDICAL STABILITY OPERATIONS COURSE |
MSOC is a three-day operational level course designed to prepare military healthcare professionals (E7-E9 and O3-O6) to serve as part of the military health support for stability operations. MSOC is open to Tri-Service Active Duty, Reserve, Guard, and US Public Health Service medical department personnel, as well as other US Government agencies (e.g. US Coast Guard, Dept of State, USAID, etc.) involved in stability operations.
The course will familiarize DoD healthcare personnel with the complexity of military medical diplomacy within the context of U.S. strategy and international relations. Students will gain an appreciation for operational aspects and overarching strategic aspects of planning, providing, and transitioning medical support in phases of pre- through post-conflict, including failed states/nations, to maintenance of stable states/nations.
MSOC is a Mid level course to support DoDI 6000.16, Military Health Support for Stability Operations dtd 17 May 2010. The course will contain briefings and panel discussions designed to intellectually challenge students on the application of both hard and soft power in the context of stability operations. The training will be punctuated with scenario-based vignettes/exercises that reinforce briefing and course objectives.
Pre-Requisites: Recent deployment/operational experience in the stability operations environment, experience with doctrine surrounding Stability Operations or completion of the Health Medical Aspects of Stability Operations (HMASO) on-line course on JKO/AKO website: course number J4OP-US129. Case-by-case exceptions will be considered for upcoming/planned deployments.
Gain knowledge & skills Students will leave the course with the capability to provide doctrinally sound, operationally integrated leadership and support in the military health system for stability operations that successfully meets current demands on health service support units. They will be able to better appreciate military MSO mission requirements throughout all phases of conflict and across the range of military operations, including in combat and non-combat environments.

Military Medical Humanitarian Assistance Course

|
The Military Medical Humanitarian Assistance Course (MMHAC) is a two-day interactive course designed to train U.S. military health care providers to deliver optimal medical care to civilian populations, primarily children that comprise the group most at-risk of death, in the aftermath of humanitarian emergencies.
The goal of this course is to familiarize clinicians with the unique aspects of humanitarian missions, so that they are best prepared to actively participate and lead future missions. Though the health issues may be predictable, the paradigm presents issues that rapidly progress. Resources are typically more limited than in other operations, complicating immediate intervention.
The course emphasizes practical skills and techniques, not often addressed in the curriculum of the American medical education system that will be useful to the provider who is challenged to provide the best possible medical care in an austere environment.
During the training, the didactic lectures are combined with scenario exercises and case management discussions to reinforce the key concepts. The course culminates with several hours spent in skill stations, in which participants will be challenged to manage clinical cases with the logistical and philosophical challenges of real world settings.
At the conclusion of this course, participants will be able to recognize the major causes of mortality among at-risk populations during complex humanitarian emergencies and other post-disaster settings, and demonstrate prevention and treatment strategies.

TRI-SERVICE CHEMICAL, BIOLOGICAL, RADIOLOGICAL, NUCLEAR,
& HIGH-YIELD EXPLOSIVES TRAINING PROGRAM
In 2002, the Joint Staff and the Deputy Assistant Secretary of Defense for Health Affairs-Force Health Protection and Readiness tasked DMRTI to develop Chemical/Biological (CB) medical training requirements and assess the effectiveness of the training with rigorous proficiency metrics and standards. The Tri-Service Chemical, Biological, Radiological, Nuclear, & High-Yield Explosives (CBRNE) Training Program established Standards of Competency based on corps (i.e. medical, dental, medics, etc). Emergency Preparedness and Response Course (EPRC) meets the training requirement. EPRC consists of four levels (Basic Awareness, Operator/Responder, Clinician’s, and Executive/Commander’s Course) targeted to a student’s profession/job description. Initial training should be completed within 12 months of being assigned to first permanent duty station. PUBLIC HEALTH EMERGENCY MANAGEMENT (PHEM) COURSE |
Background
In May 2003, the Deputy Secretary of Defense signed out Department of Defense Directive (DoDD) 6200.3, Emergency Health Powers on Military Installations. This DoDD created the position of the Public Health Emergency Officer (PHEO).
In March 2010, the Undersecretary of Defense for Personnel and Readiness signed out Department of Defense Instruction (DoDI) 6200.03, Public Health Emergency Management within the Department of Defense (DoD). This DoDI replaced DoDD 6200.03 and clarified the roles of the responsibilities of PHEOs and created the position of Military Treatment Facility (MTF) Emergency Managers (MEM).
PHEM Training Program
The PHEM Training Program will provide PHEOs and MEMs with the knowledge and skills they need to ensure that the DoD is ready to respond to public health emergencies and assist civilian and host nation authorities when called upon. This standardized, joint offered will ensure that PHEOs and MEMs can operate in a variety of environment including other Service’s installation and facilities, an issue of increasing concern due to Joint Basing Initiatives.
Upon completion of the training, personnel should have the knowledge and/or training to enable them to perform critical tasks needed to meet real-world requirements.
The PHEM Training Program consists of pre-requisite training, a five-day in-residence course, and a continuing education program that includes several courses currently offered by DMRTI as well as Service and Combatant Command continuing education opportunities.

HOMELAND SECURITY MEDICAL PROFESSIONALS COURSE (HLSMPC) |
Formerly known as the Homeland Security Medical Executive Course (HLSMEC)
This program focuses on working within the National Response Framework to prepare, respond, and recover from an all-hazards incident. Participants gain knowledge & skills using expert presentations, table-top exercises & professional interactions. Major speakers are representatives from various military and governmental agencies including Federal Emergency Management Agency (FEMA), Federal Bureau of Investigation (FBI), U.S. Public Health Service (USPHS), Department of Homeland Security (DHS), Department of Health and Human Services (DHHS), and the U.S. Northern Command (US NORTHCOM). State and local topics are presented by State and local emergency management and Public Health officials. Additionally, we invite a leading industry partner to discuss corporate preparedness and integration efforts locally and at the State and national levels.
At the conclusion of this course attendees will be able to identify key components that build the foundation for an effective medical response to an all-hazards incident, and identify the process to ensure that a comprehensive national medical response system is brought together and it coordinates all necessary response assets quickly and effectively to an all-hazards incident.
Intended audience: HLSMPC is open to DoD Active Duty/Reserve/Guard, US Public Health Service and US Coast Guard medical department officers and local, state, and federal healthcare professionals and emergency managers. Students in previous courses included designated command surgeons and senior staff/deputies, senior staff (doctors, nurses, admin, operations, logistics, public health/preventative med), NDMS/FCC staff, Medical Emergency Operations Center (EOC) staff, Joint Regional Medical Planners, ESF-8 related personnel, and other Emergency Preparedness officials.
Each participant must complete the following courses prior to HLSMPC attendance: FEMA IS-100: Introduction to Incident Command System; FEMA IS-700: National Incident Management System (NIMS), an introduction; FEMA IS-800: National Response Framework, an introduction. This requirement may be met by attending IS402-Incident Command System (ICS) for Executives and Senior Officials and IS 800 - National Response Framework during the pre-course session.
FEMA IS-200: ICS for Single Resources and Initial Action Incidents and Defense Support to Civil Authorities (DSCA), Phase I are highly recommended. FEMA courses are available at http://training.fema.gov/IS/. DSCA Phase I course is available at http://www.usarnorth.org.

Homeland Security Presidential Directive/HSPD-5 requires all Federal Departments and Agencies to adopt National Incident Management System (NIMS) for domestic incident management. NIMS and the National Response Framework (NRF) establish an all-discipline, all-hazards approach to the management of domestic incidents.
On 29 November 2005, the Deputy Secretary of Defense signed a memo, Implementation of the National Response Framework and National Response Plan, which requires the Department of Defense (DOD) to fully implement the NIMS requirements. This includes the training requirements which based on the individual needs according to his or her level of responsibility during a multijurisdictional, multiagency, multidiscipline incident. Each of Services has assigned the level of training that is required for assigned personnel.

ICS-402: Incident Command System (ICS) Overview for Executives/Senior Officials
This course is intended to provide a forum to discuss the agency’s strategic and executive level preparedness and response issues and challenges related to WMD/terrorism and other incidents of national significance. It is also intended to serve as a vehicle to share proven strategies and practices as well as enhanced teamwork and coordination among the agency’s senior officials responsible for emergency response to a major incident. The target audience for this course is elected officials, senior executive, senior managers and agency administrators with policy responsibilities, but without specific ICS or Multi-Agency Coordination System function/roles or responsibilities.

Military Medical Humanitarian Assistance Course

Military Medical Humanitarian Assistance Course (MMHAC) is a two-day interactive course offered by DMRTI and the Uniformed Services University of the Health Sciences (USUHS). It is designed to train U.S. military healthcare providers to deliver optimal medical care to civilian populations, primarily women and children, in the aftermath of humanitarian emergencies. This course addresses the need to prepare medical officers with the necessary skills, knowledge, and confidence to participate in such missions.
Given the military’s increasing involvement in Military Operations Other Than War (MOOTW), the focus of this course is centered on familiarizing clinicians with the unique aspects of humanitarian missions. Though the health issues are often predictable, the paradigm presents issues that rapidly progress to the severest degree. Resources are typically more limited than in other operations, complicating any attempt of immediate intervention.
MMHAC emphasizes practical skills and techniques not often addressed in the curriculum of American medical education. These skills and techniques will be useful to the provider who is challenged to provide the best possible medical care in an austere environment.
During the training, the didactic lectures are combined with scenario exercises and case management discussions to reinforce the key concepts. The MMHAC course culminates with several hours spent in skill stations where participants will be challenged to manage clinical cases with the logistical and philosophical challenges of real world settings.
The faculty is committed to the quality and credibility of this educational experience. Thus, all clinical instructors have not only mastered the clinical material, but have had personal experience practicing medicine in an austere health environment. All of the clinical cases are derived from real experiences in operational medicine.

Pre-Hospital Trauma Life Support |
PHTLS is a 2 or 2 ½-day course sponsored by the American College of Surgeons © and the National Association of Emergency Medical Technicians. PHTLS is conducted in conjunction with the Combat Casualty Care Course (C-4) and during exportable training sessions.
DMRTI supports a program that sets up a unit as a PHTLS Training Site, allowing unit personnel to sustain readiness training without dependence on DMRTI returning to the unit. The target population is tri-service medical personnel in active duty or reserve status.
The purpose of the PHTLS course is to provide the student with the underlying beliefs that are the foundation of the program - that definitive care cannot be provided for the critically injured trauma patient in the field. The PHTLS program attempts to increase each participant's understanding and skills in pre-hospital trauma management. This increased understanding of kinematics, pathophysiology, systemic impact, and intervention techniques will result in improving the assessment and treatment of the multi-system trauma patient and offer a perspective into the patient's individual needs. The PHTLS program enhances the philosophy that care of the multi-system trauma patient is unique and requires specific needs that may exceed traditional modalities. The purpose of TNCC is to present a basic level and psychomotor skills associated with implementing the trauma nursing process. TRI-SERVICE CHEMICAL, BIOLOGICAL, RADIOLOGICAL, NUCLEAR,
& HIGH-YIELD EXPLOSIVES TRAINING PROGRAM
In 2002, the Joint Staff and the Deputy Assistant Secretary of Defense for Health Affairs-Force Health Protection and Readiness tasked DMRTI to develop Chemical/Biological (CB) medical training requirements and assess the effectiveness of the training with rigorous proficiency metrics and standards. The Tri-Service Chemical, Biological, Radiological, Nuclear, & High-Yield Explosives (CBRNE) Training Program established Standards of Competency based on corps (i.e. medical, dental, medics, etc). A modular web-based distributive learning product titled Emergency Preparedness and Response Course (EPRC) meets the training requirement. EPRC consists of four levels (Basic Awareness, Operator/Responder, Clinician’s, and Executive/Commander’s Course) targeted to a student’s profession/job description. To meet this requirement, each of the services has directed their personnel to complete the Emergency Preparedness and Response Course (EPRC), a web-based distance-learning product. The program is divided into Basic, Operator/Responder, Clinician, and Executive components. Initial training should be completed within 12 months of being assigned to first permanent duty station. Emergency Medical Response to Weapons of Mass Destruction
The Emergency Medical Response to Weapons of Mass Destruction (EMRWMD) develops and sustains CBRNE competencies for DoD medical personnel whether they are assigned to an operational assignment, civilian mass casualty or humanitarian disaster response. EMRWMD better prepares military and interagency medical personnel for augmentation and sustainment to local, state, and federal authorities in response to a domestic or overseas incident.
Coordination of this course is through a partnership between DMRTI and the 2nd Medical Brigade. Oversight of the EMRWMD course will by provided by DMRTI while 2nd Medical Brigade will be the executive agency. EMRWMD is a three-day course, held at various locations with the main training installation being at Camp Parks, California. The course is divided into didactic classroom lectures, field training exercise, and table-top exercise. Topics include chemical agents, biological agents, radiological materials, high-yield explosives, triage and field management of casualties, psychological/behavioral response, decontamination, personal protection, and consequence/risk management.
Each of these topics is
re-explored in field training exercises and table top exercises where the participants utilize their knowledge through hands-on applications.

The (EPRC) Emergency Preparedness Responders Course is designed to personnel assigned to medical units, both fixed facility and operational, to effectively manage casualties during an all-hazards incident including those emanating from chemical, biological, radiological, nuclear, or high-yield explosives (CBRNE) sources. This course also explains the history and current threat of CBRNE use, the characteristics of threat agents, the pathophysiology and treatment of agent exposure, and the principles of management of threat agent casualties.
EPRC meets the Tri-Service CBRNE Medical Training Program requirements and is targeted toward the student’s profession/job description. The following courses are provided through mobile teams providing an alternative education opportunity for unit commanders.

Hospital Incident Command System (HICS) is an incident command system designed for hospitals and intended for use in both emergency and non-emergency situations. It provides hospitals of all sizes with tools needed to advance their emergency preparedness and response capability—both individually and as members of the broader response community.
HICS is a 2-day course that provides attendees the tools necessary to ensure the safety of patients, personnel, the facility, and ensure business continuity, financial recovery, as well restoration of “normal” operations.
Compliance with NIMS is a condition for any healthcare organization receiving Federal assistance.

COMBAT CASUALTY CARE COURSE (C4)
C4 is a basic 8-day tri-service continuing medical education program designed to enhance the operational medical readiness skills of physicians, nurses, physician assistants, dentists, and other specialty healthcare providers intended for initial entry medical professionals with minimal field exposure. C4 prepares junior tri-service medical officers with the knowledge critical in conducting Level I & II healthcare operations in an austere, combat environment. C4 provides training in leadership skills, field medical knowledge and the practical information needed for direct medical support of tactical units under combat conditions. The course includes 3 days of professional programs at the Defense Medical Readiness Training Institute, Fort Sam Houston and five-days of intensive field training at Camp Bullis, Texas.
Students attending C4 have the opportunity to receive certification in Advanced Trauma Life Support® (physicians and oral surgeons; dentists, physician assistants,and nurse practitioners may attend as auditors), the Trauma Nursing Core Course (nurses) and Pre-Hospital Trauma Life Support (other healthcare providers). Participants are awarded continuing education/credentials upon successful completion of these courses.
At the conclusion of the professional programs, the course moves to the field as students rotate through various training stations in an operational environment. Areas of instruction are varied and complex, ranging from bandaging and splinting to Level 1 & Level 2 care in the Tactical Simulator for Military Medicine (Tac-SiMM). Students encounter scenarios simulating combat environments where they must perform the duties of their respective career-field positions as well as the duties of first responders. They also have exposure to various real-world scenarios, as well as a myriad of medical readiness topics taught through didactic lectures.

EMERGENCY WAR SURGERY COURSE |
The Emergency War Surgery Course (EWSC), formerly known as the Trauma Refresher Course For Surgeons (TRCS), establishes combat trauma training competencies and coordinates training to develop and sustain DoD trauma surgeons whether located in an operational environment, military MTF or at a Level I trauma treatment facility.
Intended for general and orthopedic surgeons, the course was developed in response to 1991 Gulf War reports that surgeons and other healthcare providers lacked sufficient training and experience in trauma surgery. To remedy this, the Deputy Assistant Secretary of Defense for Health Affairs designated proficiency in combat surgery training as an “essential mission.” The Combat Trauma Surgical Committee (CTSC) was created in 1996 to meet this mission and to coordinate training efforts to develop and sustain DoD trauma care providers.
The EWSC is a standardized, measurable and reproducible training program that will give surgeons and other health providers exposure to critical life saving skills necessary to sustain the force, fight and win in the 21st century.

Formerly known as the Homeland Security Medical Executive Course (HLSMEC)
This program focuses on working within the National Response Framework to prepare, respond, and recover from an all-hazards incident. Participants gain knowledge & skills using expert presentations, table-top exercises & professional interactions. Major speakers are representatives from various military and governmental agencies including Federal Emergency Management Agency (FEMA), Federal Bureau of Investigation (FBI), U.S. Public Health Service (USPHS), Department of Homeland Security (DHS), Department of Health and Human Services (DHHS), and the U.S. Northern Command (US NORTHCOM). State and local topics are presented by State and local emergency management and Public Health officials. Additionally, we invite a leading industry partner to discuss corporate preparedness and integration efforts locally and at the State and national levels.
At the conclusion of this course attendees will be able to identify key components that build the foundation for an effective medical response to an all-hazards incident, and identify the process to ensure that a comprehensive national medical response system is brought together and it coordinates all necessary response assets quickly and effectively to an all-hazards incident.
Intended audience: HLSMPC is open to DoD Active Duty/Reserve/Guard, US Public Health Service and US Coast Guard medical department officers and local, state, and federal healthcare professionals and emergency managers. Students in previous courses included designated command surgeons and senior staff/deputies, senior staff (doctors, nurses, admin, operations, logistics, public health/preventative med), NDMS/FCC staff, Medical Emergency Operations Center (EOC) staff, Joint Regional Medical Planners, ESF-8 related personnel, and other Emergency Preparedness officials.
Each participant must complete the following courses prior to HLSMPC attendance: FEMA IS-100: Introduction to Incident Command System; FEMA IS-700: National Incident Management System (NIMS), an introduction; FEMA IS-800: National Response Framework, an introduction. This requirement may be met by attending IS402-Incident Command System (ICS) for Executives and Senior Officials and IS 800 - National Response Framework during the pre-course session.

JOMMC familiarizes and develops senior healthcare personnel to serve as Joint Task Force Surgeons’ and Staff members/. The mission of these individuals is to:
- Provide Health Service Support (HSS) recommendations to the JTF Commander

JOMMC is made up of just-in-time training to provide Joint HSS instruction punctuated with scenario-based tabletop exercises that reinforce course objectives.

Military Medical Humanitarian Assistance Course

Military Medical Humanitarian Assistance Course (MMHAC) is a two-day interactive course offered by DMRTI and the Uniformed Services University of the Health Sciences (USUHS). It is designed to train U.S. military healthcare providers to deliver optimal medical care to civilian populations, primarily women and children, in the aftermath of humanitarian emergencies. This course addresses the need to prepare medical officers with the necessary skills, knowledge, and confidence to participate in such missions.
Given the military’s increasing involvement in Military Operations Other Than War (MOOTW), the focus of this course is centered on familiarizing clinicians with the unique aspects of humanitarian missions. Though the health issues are often predictable, the paradigm presents issues that rapidly progress to the severest degree. Resources are typically more limited than in other operations, complicating any attempt of immediate intervention.
MMHAC emphasizes practical skills and techniques not often addressed in the curriculum of American medical education. These skills and techniques will be useful to the provider who is challenged to provide the best possible medical care in an austere environment.
During the training, the didactic lectures are combined with scenario exercises and case management discussions to reinforce the key concepts. The MMHAC course culminates with several hours spent in skill stations where participants will be challenged to manage clinical cases with the logistical and philosophical challenges of real world settings.
The faculty is committed to the quality and credibility of this educational experience. Thus, all clinical instructors have not only mastered the clinical material, but have had personal experience practicing medicine in an austere health environment. All of the clinical cases are derived from real experiences in operational medicine.

TRI-SERVICE CHEMICAL, BIOLOGICAL, RADIOLOGICAL, NUCLEAR,
& HIGH-YIELD EXPLOSIVES TRAINING PROGRAM
In 2002, the Joint Staff and the Deputy Assistant Secretary of Defense for Health Affairs-Force Health Protection and Readiness tasked DMRTI to develop Chemical/Biological (CB) medical training requirements and assess the effectiveness of the training with rigorous proficiency metrics and standards. The Tri-Service Chemical, Biological, Radiological, Nuclear, & High-Yield Explosives (CBRNE) Training Program established Standards of Competency based on corps (i.e. medical, dental, medics, etc). A modular web-based distributive learning product titled Emergency Preparedness and Response Course (EPRC) meets the training requirement. EPRC consists of four levels (Basic Awareness, Operator/Responder, Clinician’s, and Executive/Commander’s Course) targeted to a student’s profession/job description. To meet this requirement, each of the services has directed their personnel to complete the Emergency Preparedness and Response Course (EPRC), a web-based distance-learning product. The program is divided into Basic, Operator/Responder, Clinician, and Executive components. Initial training should be completed within 12 months of being assigned to first permanent duty station. Emergency Medical Response to Weapons of Mass Destruction
The Emergency Medical Response to Weapons of Mass Destruction (EMRWMD) develops and sustains CBRNE competencies for DoD medical personnel whether they are assigned to an operational assignment, civilian mass casualty or humanitarian disaster response. EMRWMD better prepares military and interagency medical personnel for augmentation and sustainment to local, state, and federal authorities in response to a domestic or overseas incident.
Coordination of this course is through a partnership between DMRTI and the 2nd Medical Brigade. Oversight of the EMRWMD course will by provided by DMRTI while 2nd Medical Brigade will be the executive agency. EMRWMD is a three-day course, held at various locations with the main training installation being at Camp Parks, California. The course is divided into didactic classroom lectures, field training exercise, and table-top exercise. Topics include chemical agents, biological agents, radiological materials, high-yield explosives, triage and field management of casualties, psychological/behavioral response, decontamination, personal protection, and consequence/risk management.
Each of these topics is
re-explored in field training exercises and table top exercises where the participants utilize their knowledge through hands-on applications.

Emergency Preparedness Response Course – (Exportable)
The (EPRC) Emergency Preparedness Responders Course is designed to personnel assigned to medical units, both fixed facility and operational, to effectively manage casualties during an all-hazards incident including those emanating from chemical, biological, radiological, nuclear, or high-yield explosives (CBRNE) sources. This course also explains the history and current threat of CBRNE use, the characteristics of threat agents, the pathophysiology and treatment of agent exposure, and the principles of management of threat agent casualties.

COMBAT CASUALTY CARE COURSE (C4)
C4 is a basic 8-day tri-service continuing medical education program designed to enhance the operational medical readiness skills of physicians, nurses, physician assistants, dentists, and other specialty healthcare providers intended for initial entry medical professionals with minimal field exposure. C4 prepares junior tri-service medical officers with the knowledge critical in conducting Level I & II healthcare operations in an austere, combat environment. C4 provides training in leadership skills, field medical knowledge and the practical information needed for direct medical support of tactical units under combat conditions. The course includes 3 days of professional programs at the Defense Medical Readiness Training Institute, Fort Sam Houston and five-days of intensive field training at Camp Bullis, Texas.
Students attending C4 have the opportunity to receive certification in Advanced Trauma Life Support® (physicians and oral surgeons; dentists, physician assistants,and nurse practitioners may attend as auditors), the Trauma Nursing Core Course (nurses) and Pre-Hospital Trauma Life Support (other healthcare providers). Participants are awarded continuing education/credentials upon successful completion of these courses.
At the conclusion of the professional programs, the course moves to the field as students rotate through various training stations in an operational environment. Areas of instruction are varied and complex, ranging from bandaging and splinting to Level 1 & Level 2 care in the Tactical Simulator for Military Medicine (Tac-SiMM). Students encounter scenarios simulating combat environments where they must perform the duties of their respective career-field positions as well as the duties of first responders. They also have exposure to various real-world scenarios, as well as a myriad of medical readiness topics taught through didactic lectures.

EMERGENCY WAR SURGERY COURSE |
The Emergency War Surgery Course (EWSC), formerly known as the Trauma Refresher Course For Surgeons (TRCS), establishes combat trauma training competencies and coordinates training to develop and sustain DoD trauma surgeons whether located in an operational environment, military MTF or at a Level I trauma treatment facility.
Intended for general and orthopedic surgeons, the course was developed in response to 1991 Gulf War reports that surgeons and other healthcare providers lacked sufficient training and experience in trauma surgery. To remedy this, the Deputy Assistant Secretary of Defense for Health Affairs designated proficiency in combat surgery training as an “essential mission.” The Combat Trauma Surgical Committee (CTSC) was created in 1996 to meet this mission and to coordinate training efforts to develop and sustain DoD trauma care providers.
The EWSC is a standardized, measurable and reproducible training program that will give surgeons and other health providers exposure to critical life saving skills necessary to sustain the force, fight and win in the 21st century.

HOMELAND SECURITY MEDICAL PROFESSIONALS COURSE (HLSMPC) |
Formerly known as the Homeland Security Medical Executive Course (HLSMEC)
This program focuses on working within the National Response Framework to prepare, respond, and recover from an all-hazards incident. Participants gain knowledge & skills using expert presentations, table-top exercises & professional interactions. Major speakers are representatives from various military and governmental agencies including Federal Emergency Management Agency (FEMA), Federal Bureau of Investigation (FBI), U.S. Public Health Service (USPHS), Department of Homeland Security (DHS), Department of Health and Human Services (DHHS), and the U.S. Northern Command (US NORTHCOM). State and local topics are presented by State and local emergency management and Public Health officials. Additionally, we invite a leading industry partner to discuss corporate preparedness and integration efforts locally and at the State and national levels.
At the conclusion of this course attendees will be able to identify key components that build the foundation for an effective medical response to an all-hazards incident, and identify the process to ensure that a comprehensive national medical response system is brought together and it coordinates all necessary response assets quickly and effectively to an all-hazards incident.
Intended audience: HLSMPC is open to DoD Active Duty/Reserve/Guard, US Public Health Service and US Coast Guard medical department officers and local, state, and federal healthcare professionals and emergency managers. Students in previous courses included designated command surgeons and senior staff/deputies, senior staff (doctors, nurses, admin, operations, logistics, public health/preventative med), NDMS/FCC staff, Medical Emergency Operations Center (EOC) staff, Joint Regional Medical Planners, ESF-8 related personnel, and other Emergency Preparedness officials.
Each participant must complete the following courses prior to HLSMPC attendance: FEMA IS-100: Introduction to Incident Command System; FEMA IS-700: National Incident Management System (NIMS), an introduction; FEMA IS-800: National Response Framework, an introduction. This requirement may be met by attending IS402-Incident Command System (ICS) for Executives and Senior Officials and IS 800 - National Response Framework during the pre-course session.
FEMA IS-200: ICS for Single Resources and Initial Action Incidents and Defense Support to Civil Authorities (DSCA), Phase I are highly recommended. FEMA courses are available at http://training.fema.gov/IS/. DSCA Phase I course is available at http://www.usarnorth.org.

JOINT OPERATIONS MEDICAL MANAGERS COURSE |
JOMMC familiarizes and develops senior healthcare personnel to serve as Joint Task Force Surgeons’ and Staff members/. The mission of these individuals is to:
- Provide Health Service Support (HSS) recommendations to the JTF Commander
- Coordinate HSS operations in the Joint Operations Area
- Establish and maintain operational medical facilities
- Coordinate Coalition and Civilian medical interoperability
JOMMC is made up of just-in-time training to provide Joint HSS instruction punctuated with scenario-based tabletop exercises that reinforce course objectives.
JOMMC is intended to provide entry level instruction on Joint HSS. JOMMC is not intended for those students who have previously attended the JTF Surgeon Course or Joint Medical Planners Course (JMPC).
JOMMC is open to Tri-Service Active Duty, Reserve, Guard, US Public Health Service, and US Coast Guard medical department personnel, as well as foreign national medical department officers. EMERGENCY MANAGEMENT COURSES

Homeland Security Presidential Directive/HSPD-5 requires all Federal Departments and Agencies to adopt National Incident Management System (NIMS) for domestic incident management. NIMS and the National Response Framework (NRF) establish an all-discipline, all-hazards approach to the management of domestic incidents.
On 29 November 2005, the Deputy Secretary of Defense signed a memo, Implementation of the National Response Framework and National Response Plan, which requires the Department of Defense (DOD) to fully implement the NIMS requirements. This includes the training requirements which based on the individual needs according to his or her level of responsibility during a multijurisdictional, multiagency, multidiscipline incident. Each of Services has assigned the level of training that is required for assigned personnel.
The Emergency Management Institute (EMI), located at the National Emergency Training Center in Emmitsburg, MD, offers a broad range of NIMS-related training, on-line and in-resident. For further information on EMI, go to http://training.fema.gov/EMI.

This course is intended to provide a forum to discuss the agency’s strategic and executive level preparedness and response issues and challenges related to WMD/terrorism and other incidents of national significance. It is also intended to serve as a vehicle to share proven strategies and practices as well as enhanced teamwork and coordination among the agency’s senior officials responsible for emergency response to a major incident. Though the health issues are often predictable, the paradigm presents issues that rapidly progress to the severest degree. Resources are typically more limited than in other operations, complicating any attempt of immediate intervention.
MMHAC emphasizes practical skills and techniques not often addressed in the curriculum of American medical education. These skills and techniques will be useful to the provider who is challenged to provide the best possible medical care in an austere environment.
During the training, the didactic lectures are combined with scenario exercises and case management discussions to reinforce the key concepts. The MMHAC course culminates with several hours spent in skill stations where participants will be challenged to manage clinical cases with the logistical and philosophical challenges of real world settings.
The faculty is committed to the quality and credibility of this educational experience. Thus, all clinical instructors have not only mastered the clinical material, but have had personal experience practicing medicine in an austere health environment. All of the clinical cases are derived from real experiences in operational medicine.

Pre-Hospital Trauma Life Support |
PHTLS is a 2 or 2 ½-day course sponsored by the American College of Surgeons © and the National Association of Emergency Medical Technicians. PHTLS is conducted in conjunction with the Combat Casualty Care Course (C-4) and during exportable training sessions.
DMRTI supports a program that sets up a unit as a PHTLS Training Site, allowing unit personnel to sustain readiness training without dependence on DMRTI returning to the unit. The target population is tri-service medical personnel in active duty or reserve status.
The purpose of the PHTLS course is to provide the student with the underlying beliefs that are the foundation of the program - that definitive care cannot be provided for the critically injured trauma patient in the field. The PHTLS program attempts to increase each participant's understanding and skills in pre-hospital trauma management. This increased understanding of kinematics, pathophysiology, systemic impact, and intervention techniques will result in improving the assessment and treatment of the multi-system trauma patient and offer a perspective into the patient's individual needs. The PHTLS program enhances the philosophy that care of the multi-system trauma patient is unique and requires specific needs that may exceed traditional modalities. TRAUMA NURSING CARE COURSE |
TNCC is 2 1/2 day course sponsored by the Emergency Nurses Association. The target population is military Nurse Corps Officers.
The purpose of TNCC is to present a basic level and psychomotor skills associated with implementing the trauma nursing process. The psychomotor skill stations facilitate initial integration of psychomotor abilities in a setting that stimulates trauma patient situations. It is the intent of the TNCC to enhance the nurse's ability to assess, rapidly and accurately, the patient’s responses to the trauma event. It is anticipated that the use of the knowledge and skills learned in the TNCC will ultimately contribute to a decrease in the morbidity and mortality associated with trauma. |

TRI-SERVICE CHEMICAL, BIOLOGICAL, RADIOLOGICAL, NUCLEAR,
& HIGH-YIELD EXPLOSIVES TRAINING PROGRAM
In 2002, the Joint Staff and the Deputy Assistant Secretary of Defense for Health Affairs-Force Health Protection and Readiness tasked DMRTI to develop Chemical/Biological (CB) medical training requirements and assess the effectiveness of the training with rigorous proficiency metrics and standards. The Tri-Service Chemical, Biological, Radiological, Nuclear, & High-Yield Explosives (CBRNE) Training Program established Standards of Competency based on corps (i.e. medical, dental, medics, etc). A modular web-based distributive learning product titled Emergency Preparedness and Response Course (EPRC) meets the training requirement. EPRC consists of four levels (Basic Awareness, Operator/Responder, Clinician’s, and Executive/Commander’s Course) targeted to a student’s profession/job description. To meet this requirement, each of the services has directed their personnel to complete the Emergency Preparedness and Response Course (EPRC), a web-based distance-learning product. The program is divided into Basic, Operator/Responder, Clinician, and Executive components. Initial training should be completed within 12 months of being assigned to first permanent duty station. Emergency Medical Response to Weapons of Mass Destruction
The Emergency Medical Response to Weapons of Mass Destruction (EMRWMD) develops and sustains CBRNE competencies for DoD medical personnel whether they are assigned to an operational assignment, civilian mass casualty or humanitarian disaster response. EMRWMD better prepares military and interagency medical personnel for augmentation and sustainment to local, state, and federal authorities in response to a domestic or overseas incident.
Coordination of this course is through a partnership between DMRTI and the 2nd Medical Brigade. Oversight of the EMRWMD course will by provided by DMRTI while 2nd Medical Brigade will be the executive agency. EMRWMD is a three-day course, held at various locations with the main training installation being at Camp Parks, California. The course is divided into didactic classroom lectures, field training exercise, and table-top exercise. Topics include chemical agents, biological agents, radiological materials, high-yield explosives, triage and field management of casualties, psychological/behavioral response, decontamination, personal protection, and consequence/risk management.
Each of these topics is
re-explored in field training exercises and table top exercises where the participants utilize their knowledge through hands-on applications.

Emergency Preparedness Response Course – (Exportable)
The (EPRC) Emergency Preparedness Responders Course is designed to personnel assigned to medical units, both fixed facility and operational, to effectively manage casualties during an all-hazards incident including those emanating from chemical, biological, radiological, nuclear, or high-yield explosives (CBRNE) sources. This course also explains the history and current threat of CBRNE use, the characteristics of threat agents, the pathophysiology and treatment of agent exposure, and the principles of management of threat agent casualties.
EPRC meets the Tri-Service CBRNE Medical Training Program requirements and is targeted toward the student’s profession/job description. The following courses are provided through mobile teams providing an alternative education opportunity for unit commanders.
- Emergency Preparedness Response Course - Basic/Awareness Course – 2 hours
- For non-medical/non-security DOD civilian personnel to include housekeeping, office workers and facility workers.
- Emergency Preparedness Response Course – Short Course (Sustainment/Refresher) – 1 day
- For medical DoD civilian and military personnel assigned to medical units, both fixed and operational.
- Emergency Preparedness Response Course – Executive Commander – 1 day
- For military executives and commanders or those assigned emergency management responsibilities

HOSPITAL INCIDENT COMMAND SYSTEM COURSE

Hospital Incident Command System (HICS) is an incident command system designed for hospitals and intended for use in both emergency and non-emergency situations. It provides hospitals of all sizes with tools needed to advance their emergency preparedness and response capability—both individually and as members of the broader response community.
HICS is a 2-day course provides attendees the tools necessary to ensure the safety of patients, personnel, and the facility, ensure business continuity and financial recovery, as well restoration of “normal” operations.
Compliance with NIMS is a condition for any healthcare organization receiving Federal assistance.
- Using ICS concepts and principles enables organizations to meet one component of NIMS compliance and promotes collaborative participation in a larger, national system.
- NIMS promotes a coordinated effort among all primary and secondary response agencies to better prevent, prepare for, respond to, and recover from events and incidents.
- U.S. Army Medical Command (MEDCOM) and Bureau of Medicine and Surgery (BUMED) have incorporated HICS into their emergency management programs comply with The Joint Commission Standards.
