Application Process Checklist
General assistance with the application process is available if you decide to pursue a pharmacist job vacancy with the Indian Health Service. Below are the three career path options and an application checklist for each option.
Federal Civil Service Application Checklist
Although the federal government does not require a standard application form for most jobs, the Indian Health Service requires certain information to evaluate your qualifications and determine if you meet legal requirements for federal employment.
- Your current Curriculum Vitae (CV)
- If claiming Indian Preference, you must submit the BIA Form 4432 “Verification of Indian Preference for Employment in BIA and IHS.”
- If claiming Veteran’s Preference, you must submit a copy of the DD-214 Form. If you claim 10-point Veteran’s Preference, you must also submit an SF-15 Form.
- A copy of the latest Personnel Action (SF-50) if a current or former federal employee and/or if requesting reinstatement eligibility
- If you are a current federal employee, you must submit a copy of your most recent performance appraisal.
- Transcripts must be submitted if requested in the job announcement or if you are substituting education for experience.
- A signed Addendum to Declaration for Federal Employment (OF-306) Indian Health Service Child Care & Indian Child Care Worker Positions may be required. (Note: You will be required to complete a security questionnaire and fingerprint chart for investigative purposes under the PL 101-630 Indian Child Protection and Family Violence Prevention Act.)*
- Declaration for Federal Employment (Form OF-306)
- A copy of your pharmacist license
*Note: Persons who have been arrested for or charged with a crime involving a child or a violent crime against a person are not eligible for employment with IHS under PL 101-630.
USPHS Commissioned Corps Application Checklist
- Application for Appointment as a Commissioned Officer in the US Public Health Service Commissioned Corps (Form PHS-50)
- Reference Request for Applicants to the US Public Health Service Commissioned Corps (Form PHS-1813)
- Official transcripts from any post-secondary education that resulted in the granting of a degree
- Your current Curriculum Vitae (CV)
- A copy of a current professional license and/or board certifications (if licensed/board certified and/or if applicable)
- For individuals that have current or have had previous uniformed service — a copy of your last three officer or enlisted evaluations and, if applicable, a copy of a DD-214 (member-4 copy)
Direct Tribal Hire Application Checklist
There will be a formal application process for you to follow at the local level after you apply via the Direct Tribal Hire career path. The primary contact at the site will be your best source of guidance for the Tribal Hire process.
Positions listed as Tribal hire are not part of the federal personnel systems, so pay and benefits vary by Tribe. Tribal hire opportunities generally require a license in the state in which the employee will work. The only exception is Alaska, which allows pharmacists with a valid license from another US state to work for the Tribal Corporation for a predetermined period of time while they actively pursue a valid Alaska license.
