NOMINATE CONSUMER/PEER LEADERS

Consumer/Peer leader nominations are no longer being accepted for SAMHSA’s 2012 Voice Awards.

If you know of a consumer/peer leader who has been instrumental in educating the public about mental health and/or substance use disorders and made outstanding contributions in all of the following areas, please nominate him/her for a 2012 Voice Award.

This year, consideration is being given to consumer/peer leader nominees who educate the public about the behavioral health challenges of athletes and help empower them to begin the journey of recovery.

For the purposes of the Voice Awards, the term “consumer/peer leader” is defined broadly as an individual who has received or is receiving services for a mental health and/or substance use disorder.

To make a nomination, please download and complete the consumer/peer leader nomination form, and send it back to the Voice Awards team using the contact information below.

Nominations are open to anyone and are free. There is no limit to the number of nominations you can submit and self-nominations are welcome.

CONSUMER/PEER LEADER NOMINATIONS WERE DUE FRIDAY, APRIL 13, 2012.
