The CHR Data Mart provides access to core CHR reports for all sites exporting data to the National Data Warehouse and
includes the ability for senior CHR leadership and program staff to access the most critical performance data for the program including:

- CHR Number Served and Client Contacts
- Health Problem Codes by Age/Gender with Service and Travel Hours
- Service Code Reports
- Activity Location Reports
- Referred To CHR By and Referred By CHR To Reports
- Division of Behavioral Health (DBH)
- The Behavioral Health Data Mart and Reporting System provides national, regional, service unit and facility level reporting related to these health issues:
- Methamphetamine Diagnosis
- Suicide Related Events
- Behavioral Health "workload" reporting
- "Hot spots" or cluster events by community of residence
- Trend information
- Division of Environmental Health Services (DEHS)
- The Notifiable Disease and External Cause of Injury (NDECI) Data Mart and Reporting System tracks data for these five specific categories:

- any ICD9 code with the first 3 digits of "493"

Injury
- ICD9-CM Codes E800-E999 with the exception of Suicide related injuries

Intestinal Disease
- ICD9 CM codes 001-009

Notifiable Disease
- any ICD9-CM codes in the following code ranges: 001-139, 460-519, 680-709, V01-V02 and V07-V09

- The Dental Data Mart utilizes data from the Resource Patient Management System (RPMS) Patient Care Component (PCC) exports,
Contract Health Services (CHS) data received from the Fiscal Intermediary, data export from the CHS/MIS application and non-RPMS data provided through the "simplified" export file
to provide the following reports:
- Dental Visits by Month
- RVU by Month and Level Reports
- Service by Month and Level Reports
- Services per Visit and Patient Reports
- GPRA Sealants and Flouride Reports
- Early Childhood Caries (ECC) Reports
- Eye Care Data Mart
- The Eye Care data mart makes full use of data mapped and exported to the IHS’ National Data Warehouse project. For this "version 1" application, nine (9) major eye care categories have been defined. These categories include:
- Total Eye Care related encounters
- Cataracts
- Macular Degeneration
- Blindness
- Refractive encounters
- Glaucoma
- Diabetic Retinopathy
- Amblyopic
- Eye Glasses Dispensing
- National Immunization Reporting System (NIRS)
- The NIRS web-based application is designed to collect quarterly immunization data from each participating IHS facility. Data entered through this system are used to develop Area and National level immunization reports regarding the following vaccination groups:
- 3 to 27 Month Old Vaccinations
- Two Year Old Vaccinations
- Adolescent Vaccinations
- Influenza Vaccinations
- Healthcare Personnel Influenza Vaccinations
